Indonesia has successfully expanded universal health coverage (UHC) via its National Health Insurance (NHI) initiative. The secondary dataset used in this study originated from The Ministry of Health of the Republic of Indonesia's 2019 nationwide survey, encompassing the aspects of 'Abilities and Willingness to Pay, Fee, and Participant Satisfaction in implementing National Health Insurance in Indonesia'. A weighted sample of 18,514 poor people in Indonesia was the subject of the study's population. The dependent variable in the study was NHI membership. The investigation involved seven independent variables: wealth, residence, age, gender, education, employment, and marital status, which were explored in the study. Statistical results highlight a trend wherein NHI membership is more prominent among the financially disadvantaged with advanced educational qualifications, residing in urban environments, being older than 17, being married, and having higher financial stability. NHI membership among the impoverished is disproportionately higher for those with higher educational levels compared to those with lower levels of education. In predicting their NHI membership, various factors were assessed, including their place of residence, age, gender, employment status, marital status, and financial situation. Primary education, in the context of poverty, is associated with a 1454-fold increase in the likelihood of becoming an NHI member, in contrast to those without any formal education (Adjusted Odds Ratio 1454; 95% Confidence Interval 1331-1588). Categorizing and connecting physical activity (PA) with sedentary behavior (SB) is key to creating successful lifestyle interventions for the youth population. Through a systematic review (Prospero CRD42018094826), the study sought to determine the clustering characteristics of physical activity and sedentary behavior, and the contributing factors, amongst boys and girls aged 0 to 19. Five electronic databases were searched. In agreement with the authors' descriptions, two independent reviewers extracted cluster characteristics, while a third reviewer adjudicated any disagreements. Among the seventeen qualifying studies, the age range for participants spanned from six to eighteen years. Nine cluster types were identified in mixed-sex samples; boys showed twelve types and girls, ten. Female groups displayed characteristics of low physical activity and low social behavior, alongside low physical activity and high social behavior; conversely, the majority of male clusters exhibited high physical activity and high social behavior, and high physical activity accompanied by low social behavior. Sociodemographic details demonstrated a paucity of associations with all the identified clusters. In the High PA High SB clusters, a substantial link between higher BMI and obesity prevalence was detected in boys and girls, for most of the tested associations. On the contrary, individuals falling into the High PA Low SB clusters demonstrated lower BMIs, waist circumferences, and fewer cases of overweight and obesity. The distribution of PA and SB into clusters was seen to differ between boys and girls. Since 2019, Beijing municipal hospitals, in the wake of the Chinese medical system reform, spearheaded a novel pharmaceutical care model, initiating medication therapy management (MTM) services within their ambulatory care settings. China was one of the first locations where our hospital initiated this service. At the present time, there were not many reports on the impact MTMs were having in China. Our hospital's experience with implementing MTMs, alongside an exploration of the viability of pharmacist-led ambulatory MTMs, and an analysis of how MTMs impact patient medical expenditures, are presented in this investigation.
The retrospective study was carried out at a university-connected, comprehensive tertiary care hospital situated in Beijing, China. To be part of the study, patients had to have complete medical records and pharmaceutical documentation, along with receipt of at least one Medication Therapy Management (MTM) intervention between May 2019 and February 2020. Pharmacists, guided by the American Pharmacists Association's MTM standards, rendered pharmaceutical care to patients. This involved assessing the variety and volume of medication-related concerns expressed by patients, pinpointing any medication-related issues (MRPs), and formulating medication-related action plans (MAPs) to address them. A record was kept of every MRP identified by pharmacists, pharmaceutical intervention employed, and resolution recommendation made, in addition to calculating the decrease in cost for treatment drugs for patients.
Eighty-one patients, from a group of 112 who received MTMs in ambulatory care settings, whose records were complete, were chosen for inclusion in this study. Among the patients examined, 679% suffered from five or more medical conditions, and 83% of this group were taking more than five drugs simultaneously. Medication Therapy Management (MTM) procedures on 128 patients documented their perceived medication-related demands, with the assessment and evaluation of adverse drug reactions (ADRs) being the most frequently expressed need, representing 1719% of all requests. A total of 181 MRPs were identified, averaging 255 MPRs per patient. The top three MRPs were nonadherence (38%), excessive drug treatment (20%), and adverse drug events (1712%). The multifaceted care needs of residents in nursing homes, coupled with a shortage of nursing staff, present considerable difficulties for healthcare professionals. Hence, nursing homes are undergoing a transformation to become personalized home-like facilities that focus on patient-centred care. The evolution of nursing homes, and the inherent challenges, demand an interprofessional learning culture, despite a scarcity of knowledge regarding the enabling elements of its development. The facilitators are the focus of this scoping review, which aims to uncover the contributing elements to their identification.
A scoping review was undertaken using the JBI Manual for Evidence Synthesis (2020) as the guiding document. In 2020-2021, the search strategy spanned seven international databases: PubMed, Cochrane Library, CINAHL, Medline, Embase, PsycINFO, and Web of Science. Reported factors supporting an interprofessional learning culture in nursing homes were independently identified by two researchers. The extracted facilitators were inductively grouped and categorized by the researchers into distinct groups.
The comprehensive search unearthed 5747 studies. Following the identification and removal of duplicates, and the subsequent screening of titles, abstracts, and full texts, thirteen studies that matched the inclusion criteria were incorporated into this scoping review. We grouped 40 facilitators into eight clusters: (1) common language, (2) common aims, (3) distinct responsibilities and duties, (4) knowledge exchange and learning, (5) collaborative procedures, (6) change facilitation and creative support by the front-line supervisor, (7) open-mindedness, and (8) a secure, respectful, and transparent setting.
